Díky výsledkům kontrolovaných lékových studií v posledním desetiletí se posunul výběr farmakoterapie nemocných ve stabilní fázi chronického srdečního selhání (CHSS) od léků s převážně symptomatickým efektem ke skupinám léků, které kromě zlepšení hemodynamických ukazatelů a zlepšení kvality života pacientů příznivě ovlivňují i prognózu nemocných s tímto syndromem. Jsou to léky, které na různé úrovni a různou měrou zasahují do nepříznivě zvýšené neurohormonální aktivace, která syndrom CHSS provází. V současné době máme důkazy o ovlivnění mortality nemocných s CHSS pro: inhibitory angiotenzin-konvertujícího enzymu (inhibitory ACE), betablokátory a spironolakton. Čekáme výsledky studií s blokátory receptoru angiotenzinu II AT1.
