Esenciální hypertenze je dnes jen vzácnou příčinou terminálního selhání ledvin, přibývá ale starších hypertoniků nejen s ischemickou chorobou srdeční, ale i ischemickou chorobou ledvin (stenózou renální tepny). Renální hypertenze je nejčastější příčinou sekundární hypertenze. Vyskytuje se zejména u některých unilaterálních nemocí ledvin, polycystických ledvin, chronické glomerulonefritidy a u nemocných s chronickým selháním ledvin. Hypertenze u těchto nemocných zvyšuje kardiovaskulární morbiditu a mortalitu a je nejdůležitějším faktorem progrese renální insuficience. Účinná léčba hypertenze (optimálně inhibitory angiotenzin konvertujícího enzymu) je schopna progresi renální insuficience výrazně zpomalit.
