Biguanidy jsou antihyperglykemická perorální antidiabetika. Jediným derivátem, který je doporučován diabetologickými experty na celém světě, je metformin. Tento lék je lékem volby u obézních diabetiků 2. typu v monoterapii nebo v kombinaci s deriváty sulfonylurey. Jeho hlavním účinkem je zvýšení citlivosti periferních tkání na působení inzulínu, tj. snížení periferní inzulínové rezistence.
