Analgetická nefropatie je progresivním chronickým ledvinným onemocněním vyvolaným dlouhotrvajícím abúzem vícesložkových analgetik. Patologicko-anatomickým substrátem je predilekční postižení dřeně ledvin se vznikem papilárních nekróz, s rozvojem fibrózy a atrofie ledvinového parenchymu. Klinickým důsledkem je porucha funkce ledvin s tendencí ke vzniku vleklého nezvratného selhání ledvin. Analgetická nefropatie je součástí -analgetického syndromu- a je jednou z jeho pozdních manifestací. Je spojena s vyšším výskytem karcinomů z přechodného epitelu vývodných močových cest a adenokarcinomů ledvin. Léčebná opatření zahrnující přerušení užívání analgetik, léčbu hypertenze a močové infekce jsou schopná zastavit nebo zpomalit pokračující pokles ledvinných funkcí. Analgetická nefropatie je jedním z mála ledvinových onemocnění, jejichž vzniku lze zabránit preventivními opatřeními.
