Nejčastějším metabolickým onemocněním skeletu je osteoporóza, prostý úbytek kostní hmoty se zachovalou rovnováhou organické a mineralizované složky kostní hmoty. Při průkazu sníženého množství kostní hmoty (RTG, kostní denzitometrie) je prvním krokem odlišení osteoporózy od ostatních osteopatií charakterizovaných sníženou mineralizací kostní tkáně a zároveň vyloučení chorob jako možných příčin sekundární osteoporózy. Důkladné vyšetření a uplatnění širší diferenciální diagnostiky zasluhují především onemocnění, která se manifestují patologickou frakturou či významnými odchylkami v laboratorním vyšetření
