Izolovanou diastolickou dysfunkcí trpí nejméně 30% nemocných s chronickým srdečním selháním. K pochopení patofyziologie tohoto typu srdečního selhání je nezbytné se seznámit alespoň v hlavních rysech s významem diastoly. V článku jsou popsána hlavní echokardiografická kriteria posuzování diastolické dysfunkce, která jsou nezbytná pro stanovení diagnózy, dále farmakologická léčebná taktika, která je poněkud odlišná od klasické léčby chronického srdečního selhání s postižením převážně systolické funkce. Předpokládáme, že diagnóza diastolické dysfunkce se bude stále častěji objevovat v závěrečných zprávách našich pacientů, a je proto důležité, aby i praktičtí lékaři byli blíže seznámeni s touto problematikou.
