Zásady farmakologie u nemocných se sníženou funkcí ledvin

Dávkování farmak, která jsou vylučována pouze nebo převážně ledvinami u jedinců se sníženou funkcí ledvin, musí být adekvátně sníženo, nemá-li dojít k jejich akumulaci a rozvoji nežádoucích (někdy velmi závažných) komplikací. Postupy, které umožňují adekvátně upravit dávkování farmak u jedinců se sníženou renální funkcí jsou různě náročné z hlediska jejich kalkulace a závisí na potřebě přesnosti úpravy dávkování. Pochopitelně, že úprava dávkování farmak, která jsou potenciálně toxická (často nefrotoxická) již při nevelkém překročení horní terapeutické hranice, musí usilovat o větší přesnost. V následujícím přehledu rozvedeme postupy, které užíváme v běžné ambulantní praxi a postupy náročnější, které jsou většinou užívány v závažnějších případech u hospitalizovaných pacientů.
