1. Etiologie

Akutní lymfatická leukemie (ALL) je nemoc, která vzniká maligní transformací kmenové hemopoetické buňky. Tato buňka se diferencuje v lymfoblasty. Ke vzniku maligního klonu dochází na různých etážích diferenciace lymfoidních blastů, což lze zjistit vyšetřením membránových a cytoplazmatických antigenů typických pro jednotlivé podskupiny fyziologických lymfatických buněk, které jsou exprimovány na buňkách různých typů ALL. Proliferace blastů se stejně jako u jiných typů akutních leukemií vymyká fyziologické regulaci. Na úrovni blastů je diferenciace zastavena. U ALL jsou časté chromozomální translokace. Nezřídka jsou přitom přemísťovány onkogeny do blízkosti genů pro gamaglobuliny nebo pro receptor T buněk (TCR). Tímto přemístěním jsou onkogeny aktivovány.
