Nežádoucí účinky anorektik

Obezita představuje závažný rizikový faktor kardiovaskulárních onemocnění. Základem léčby obezity je změna poměru kalorického příjmu a výdeje. Podpůrnými prostředky léčby jsou anorektika, léky potlačující chuť k jídlu. Působí jednak centrálně přes katecholaminy a serotonin a jednak periferně vazbou na lipázy v gastrointestinálním traktu.

Autor upozorňuje na nežádoucí účinky některých anorektik. Fenfluramin a dexfenfluramin, zvláště v kombinaci s phenterminem, mohou zvláště při déletrvajícím užívání vyvolat chlopenní srdeční vady a/nebo plicní hypertenzi. Tato onemocnění jsou dlouhou dobu asymptomatická nebo oligosymptomatická a proto nepřivádějí nemocného k lékaři včas. I v naší populaci jsou jedinci, kteří tuto anorektickou léčbu užívali. Je nutné je pravidelně kontrolovat, cíleně pátrat po symptomech, srdečních šelestech či jiných změnách klinického stavu. Hlavní úloha v detekci těchto nemocných náleží praktickému lékaři.
