Akutní disekce aorty je život ohrožující onemocnění, které se vyskytuje nejčastěji u starších osob jako komplikace neléčené hypertenze a potažmo centrální aterosklerózy. Podstatou onemocnění je odtržení vnitřní vrstvy - tunica intima - aortální stěny krevním proudem. Její symptomatologie je velmi variabilní, závislá na rozsahu vzniklého poškození, a také intenzita jejího vedoucího příznaku - bolesti na hrudi - může kolísat. Vzhledem k tomu, že bolest na hrudi má v klinické praxi mnohem častěji jinou příčinu než právě disekci, myslí se na tuto diagnózu při prvním kontaktu málo. To však může mít pro pacienta vážné následky. V následujícím sdělení je podán stručný přehled patogenezy, diagnostických možností a klasifikace aortální disekce s ohledem na praktické léčebné kroky v první fázi onemocnění.

Aortic Dissection

Acute aortic dissection is life threatening disorder affecting mainly older people as a consequence of non-treated hypertension, as well as atherosclerosis. The hallmark of this disease is the detachment of inner layer - tunica intima - of aortic wall caused by blood stream. Its clinical signs are variable, dependent on the size of resulting damage. Intensity of the leading symptom - chest pain - can differ too. Because chest pain in clinical practice is caused mainly by other diseases than aortic dissection, this diagnosis is often omitted during first evaluation. This could lead to serious consequences for a patient. This article renders a brief review of pathogenesis, diagnostic modalities and classification of aortic dissection with regard to management of treatment during the first phase of disease.
