Mikroangiopatické komplikace diabetu

Pro diabetes mellitus (DM) jsou typické pozdní orgánové projevy postupně se vyvíjejícího trvalého poškození vaziva kloubů, šlach, kůže a stěn cév, ke kterému dochází v důsledku irreverzibilních metabolických změn. Z cévních komplikací rozlišujeme diabetickou mikroangiopatii a makroangiopatii. Makroangiopatické komplikace jsou častější u diabetiků 2. typu, mikroangiopatické u diabetiků 1. typu. Vývoj mikroangiopatických změn je podmíněn hyperglykémií, která je charakteristickým rysem diabetu. Klinicky významné jsou retinopatie a nefropatie. Mikroangiopatické změny postihují i vasa nervorum a zčásti se tak podílejí i na vzniku a rozvoji diabetické neuropatie. Diabetická nefropatie je nejčastější příčinou úmrtí u diabetiků 1. typu, diabetická retinopatie je ve vyspělých zemích nejčastější příčinou slepoty.
