Následná terapie po prodělaném ischemickém iktu

V článku jsou shrnuty současné poznatky a jednotlivé terapeutické postupy u pacientů po ischemické cévní mozkové příhodě. Správná terapie po prodělaném iktu obsahuje všechny následující postupy, tedy farmakologickou i nefarmakologickou sekundární prevenci a zároveň správnou léčbu následků iktu. V první části je probrána sekundárně preventivní péče. Podle etiopatogeneze iktu jsou zde rozebrány jednotlivé indikace antiagregační, antikoagulační a chirurgické terapie. Nezbytnou součástí sekundární prevence je diagnostika a terapie ovlivnitelných rizikových faktorů. V další části se zabýváme léčbou následků iktu, tedy především rehabilitační, logopedickou, psychologickou a symptomatickou terapií.

Management of patients after ischemic stroke

The article summarizes current knowledge and particular therapeutic approaches in management of patients after stroke. Correct management of patients after stroke consists of both pharmacological and nonpharmacological secondary prevention and, at the same time, of appropriate treatment of stroke consequences. The first part of this article concentrates onto secondary preventive care. Due to ethiopatogenesis of stroke, the indications of antiplatelet therapy, anticoagulant therapy and surgical treatment are discussed here. The essential part of the secondary prevention involves diagnosis and treatment of affectable risk factors. The other part deals with the therapy of stroke sequelae, that comprises of rehabilitation, logopedics, psychological and symptomatic therapy.
