Infekční endokarditida aneb myslíme na toto onemocnění?

Infekní endokarditida pedstavuje onemocnní, které mže mít pro pacienta fatální následky. Pokud není indikována léba operaní, je tato nemoc vtšinou spojena s nutností dlouhodobé a intenzivní antibioterapie. Popsán je pípad bakteriální endokarditidy, která by diferenciáln diagnosticky mohla být zamnna s bžnou bronchopneumonií u starší nemocné. Dominujícím vyšetením, které v tomto pípad vedlo ke zjištní správné diagnózy, byla jícnová echokardiografie. Infekní zdroj pedstavovala cévní protéza na dolní konetin. Z chirurgického hlediska by jedinou úspšnou lébou byla pouze amputace levé dolní konetiny, což však nemocná odmítla. Tato skutenost spolu s nálezem masivní bakteriální endokarditidy více chlopní je podle pedchozích prací spojena se špatnou prognózou nemocných, což se potvrdilo i v našem pípad. Z dvodu nevýrazných subjektivních potíží nemocné, za kterými se však skrývala rozsáhlá infekní endokarditida, jsme se rozhodli k publikaci následující kazuistiky.
