Kvalita života nemocných s chronickou obstrukční plicní nemocí

V článku je podán přehled o kvalitě života u nemocných s chronickou obstrukční plicní nemocí (CHOPN). Standardizované, obecné a specifické dotazníky jsou novými instrumenty hodnocení kvality života (KŽ). Mohou komplexně zhodnotit CHOPN a její dynamiku. V naší sestavě bylo 438 nemocných s CHOPN (stadium 0, I, II, III, IV), 235 mužů, 203 žen, průměrný věk 63 let. Skupina byla porovnána se 101 zdravými lidmi pomocí obecného dotazníku SF-36 a specifického dotazníku SGRQ (St. George Respiratory Questionnaire). Nejnižší skóre KŽ v SF-36 bylo u podskupiny 166 CHOPN z dlouhodobou domácí oxygenoterapií v oblasti fyzické aktivity. Skóre těchto nemocných bylo ve srovnání se zdravými signifikantně nižší, tj. horší (ANOVA, P < 0,001). Ve všech oblastech KŽ, hlavně v oblasti fyzické aktivity (10/86), v emočních problémech (31/77), vitalitě (31/72) a všeobecném hodnocení zdraví (12/72). Reprodukovatelnost SF-36 dotazníku byla po třech až pěti měsících do prvního vyplnění velmi dobrá. Substituce dotazníku SF-36 dotazníkem SGRQ byla také doložena. Hodnocení KŽ se v poslední době často používá při posuzování farmakologické a nefarmakologické léčby CHOPN.

Health related quality of life in COPD patients

The review of health-related quality of life in COPD patiens was shown. Standardised general and specific questionnaires are new instruments in the evaluation of quality of life. They may assess in a extensive complex the COPD and its dynamics. Our COPD group consisted of 438 patients, stages 0, I, II, III, IV, 235eneral questionnaire SF-36 and specific one SGRQ (St. George Respiratory Questionnaire). The lowest scores of SF-36 in a subgroup of 166 COPD with long-term home oxygen therapy in physical domains were found. SF-36 scores of this COPD group were significantly lower (ANOVA, P < 0,001) compared to healthy group in all domains: physical activity (10/86), emotional problems (31/77), vitality (31/72), general health (12/72). Reproducibility of SF-36 questionnaire was very good 3 to 5 months after first administration. Possibility of substitution of SF-36 and SGRQ was also documented. The quality of life evaluation is widely used at pharmacologic and non-pharmacologic treatment of COPD recently.
