Subklinické formy onemocnění štítné žlázy se vyskytují při nejmenším stejně často, jako formy manifestní. U funkčních poruch (subklinická hypotyreóza a subklinická tyreotoxikóza) je diagnostickým kritériem nikoli chybění klinického obrazu, ale nepoměr mezi normální hladinou tyreoidálních hormonů (tyroxin a trijodotyronin) a změněnou hladinou TSH. Volně lze do skupiny subklinických tyreopatií zařadit osoby s pozitivními tyreoidálními protilátkami (proti tyreoglobulinu a tyreoidální peroxidáze) nebo s pozitivním sonografickým nálezem (hypoechogenita) při chybění klinických známek zánětu, dále sonograficky zjištěné mírné zvětšení štítné žlázy bez pozitivního palpačního nálezu a mikrokarcinom štítné žlázy. Jsou uváděny rozdílné názory na léčbu těchto chorob. Dispenzarizace je nutná vždy, léčba je nutná v graviditě a u rizikových skupin.

Subclinical thyroidals

Subclinical forms of thyroid gland diseases are at least so frequent as those clinically manifest. In functional disorders (subclinical hypothyroidism and subclinical hyperthyroidism), a diagnostic criterion is disproportion between the normal levels of thyroidal hormones (thyroxin and triiodothyronine) and altered level of TSH and not the lack of clinical picture. In a broad sense, also people with positive thyroidal antibodies (against thyreoglubulin and thyroidal peroxidase) or with ultrasound finding (hypoechogenity) might be rank among subclinical thyroideopathies and also those without clinical sings of inflammation or with mild enlargement of thyroid gland at sonography without palpable finding and with microcarcinoma of thyroid gland. Different opinions on treatment of these diseases are presented. Dispensarisation is always necessary, treatment is necessary in pregnancy and in risk groups.
