Poporodní spontánní ruptura jater při hellp syndromu

Sdělení popisuje kazuistiku pacientky s rozvojem poporodního HELLP syndromu, komplikovaného rupturou jater, s uvedením diagnostických a terapeutických kroků. HELLP syndrom - charakterizovaný triádou Hemolysis, Elevated Liver tests, Low Platelet count - tvoří s preeklampsií a eklampsií skupinu patologických stavů souvisejících s těhotenstvím, jejichľ patofyziologickým podkladem je pravděpodobně aktivace endotelu s intravaskulární koagulací. Ruptura jater patří spolu s rozvojem DIC, abrupcí placenty, akutním selháním ledvin a nekardiálním plicním edémem k nejobávanějąím komplikacím HELLP syndromu s významnou mortalitou.
