Imunological laboratory tests in suspect of celiac disease

Celiac disease is an autoimmune enteropathy induced by the gliadin intolerance. In the sera of untreated patients, antibodies against gliadin (AGA), reticulin (ARA), endomysium (EMA) and tissue transglutaminase (anti-tTG) may be found. The aim of this study was to develop optimal algorithm for screening laboratory tests. The retrospective analysis of our celiac laboratory test results from the year 2004 was done. Both sensitivity and specificity of anti-tTG for the diagnosis of celiac disease was excellent (100%). Sensitivity and specificity AGA was considerable lower (sensitivity AGA IgA 60%, AGA IgG 70%, specificity AGA IgA 60%, AGA IgG 35%). Except celiac disease, AGA positivity was detected in numerous other diseases (Crohn’s disease, food allergies, systemic and organ specific autoimmune diseases, diabetes). According to the high anti-tTG sensitivity and specificity we recommend this test as a first screening test in adults with suspected celiac disease.
