The diagnostic work-up for thyroid disorders became advanced recently. However, it should be conducted, especially in a polymorbid patient, experienced clinician, who needs to communicate and cooperate with a laboratory specialist, because despite high sensitivity and specificity thyroid parameters are under different biological and pharmacological effects and also methodology artifact. These need to be known by indicating and therefore results reading clinician and also by laboratory staff. In the current practice, the relationship of TSH and FT4 has a dominant role, which is diagnosed be a supersensitive III. generation IMA method, in the diagnosis of functional disorders of thyroid gland. It gives 6 characteristic types of functional problems. The detection of autoantibodies against thyroid peroxidase (TPOAb), thyreoglobulin (TgAb) and TSH receptor (TRAb) has the unsubstitutable role for the confirmation of the etiology of the thyroid disorders and also routine fine-needle biopsy of the thyroid gland under ultrasound guidance with subsequent cytological evaluation of acquired sample.
