Náhle srdcové úmrtie a šport - II. časť

Mechanizmy náhleho srdcového úmrtia – Uvádzajú sa rôzne možné mechanizmy, ktorými môže intenzívna športová aktivita vyvolať/zhoršiť ischémiu myokardu či srdcovú dysrytmiu u športovcov (tlakové zmeny na úrovni koronárnych ciev, fisurácia vulnerabilného aterosklerotického plátu, oklúzia aterosklerotickým plátom, spazmus koronárnej artérie, elektroinstabilita myokardu, dysrytmie) (9, 22). Bez ohľadu na mechanizmus zodpovedný za udalosť, bezprostredne pred náhlym úmrtím počas športovania dochádza k masívnej redukcii srdcového výdaja (sekundárne buď k ischémii myokardu, alebo dysrytmii), zníženiu prietoku krvi mozgom a strate vedomia. Predpokladá sa, že kombinácia spúšťacej udalosti s vnímavým myokardom iniciuje potenciálne letálnu dysrytmiu, najmä komorovú fibriláciu. Ďalším možným vysvetlením pre náhle úmrtie tesne po skončení športového výkonu je pokles srdcového výdaja po náhlom prerušení intenzívneho športového výkonu s následnou arteriálnou vazodilatáciou...
