One of the most common endocrine diseases is primary hyperparathyrodism (PHPT). PHPT is generalized disturbances of calcium, phosphate and bone metabolism, which is caused by long time increased secretion of parathormone (PTH). PHPT is caused by increased production of PTH by adenoma of one or more parathyroid glands or by hyperplazia of parathyroid glands. Clinical picture of PHPT dramatically changed during the last years especially in states with high medical level duo to rutine estimation of calcemia, phosphatemia and iPTH. In contrast with this in our set of our patients (912) there are still patients suffering from bone, renal and metabolic complication of PHPT. Correct and fast diagnosis of PHPT has great chance to give the patients full health. Most common used method for localization of adenoma of parathyroid gland are sonography, 99mTc -MIBI, MRI or SPECT. Removing the adenoma is the first choice of treatment of patients with PHPT.

Keywords: Key words: primary hyperparathyroidism, calcemia, phosphatemia, iPTH, osteodystrophy, nefrolitiasis, asymptomatic PHPT.
