Neobvyklý průběh preneoplastické dermatomyositidy jako první projev karcinomu prostaty

V příspěvku autoři chtějí upozornit na neobvyklý začátek karcinomu prostaty s projevy dermatomyositidy, s postižením svalstva horních cest dýchacích, horní části jícnu a kůže horní poloviny hrudníku. Pacient se dostavil s dýchacími potížemi, které se rychle a téměř denně zhoršovaly, až byl nucen být urychleně, pro akutní dušnost, hospitalizován na interním oddělení. Pro nejasný původ a kritický stav byla zahájena léčba vysokými dávkami kortikoidů. Po zlepšení stavu a všestranném pátrání po diagnóze bylo zjištěno, že se jedná o karcinom prostaty. Po dalším klinickém, laboratorním, imunologickém a onkologickém vyšetření bylo na základě výsledků možno diagnostikovat dermatomyositidu jako první projev karcinomu prostaty. Mezitím progredovaly změny na kůži obličeje a hrudníku. Stav se při léčbě kortikoidy a cyklofosfamidem postupně zlepšoval. Nyní, 3,5 roku od začátku, je pacient ve velmi dobrém stavu jak po stránce klinické, tak i laboratorní.
