Hypoosmolární syndrom u nemocného s tumorem pankreatu

Autoři popisují případ 79letého muže s hypoosmolárním syndromem. Potíže nemocného byly nejdříve dávány do souvislosti s předpokládanou nepřiměřenou hypersekrecí antidiuretického hormonu při jinak asymptomatickém tumoru hlavy pankreatu. Nádor slinivky byl úspěšně chirurgicky odstraněn, patolog zhodnotil resekát jako dobře diferencovaný endokrinní tumor, ale potíže nemocného přetrvávaly dál. Při dalším vyšetření jsme u pacienta diagnostikovali panhypopituitarizmus s hyperprolaktinémií. Počítačová tomografie odhalila makroadenom hypofýzy. Substituce hypopituitarizmu a léčba dopaminergním agonistou vedly k úplné normalizaci vnitřního prostředí a potíže pacienta ustoupily. Sdělení dokládá vzácný simultánní výskyt dvou endokrinních tumorů manifestujících se ve vysokém věku.
