Granulomatosis with polyangiitis (formerly known as Wegener‘s granulomatosis) is a necrotizing vasculitis that affects the small vessels,
especially those of the respiratory system, and kidneys via the formation of granulomas, manifesting as necrotizing glomerulonephritis.
Our case report presents the case of 56 years old man, who was sent to the Department of Internal Medicine at the University Hospital
of Ostrava from a regional hospital because of a persisting intermittent fever. The patient´s clinical characteristics included bilateral
perimaleolar oedema, mild weight loss, subjective complaints of an upper respiratory tract infection lasting for several months, and
migrating pain involving the large joints. The case is highlighting the polymorphic symptomatology of this relatively rare disease.
