Therapy of venous thrombosis

MUDr. Alena Broulíková CSc
III. interní klinika VFN a 1. LF UK, Praha

The most common treatment of venous thrombosis are drugs with anticoagulant effect. Only a part of patients is treated with thrombolytic therapy and only small part is available for the surgical treatment. The heparin is the first treatment for the anticoagulant therapy. The choice is intravenous infusion with unfractionated heparin or low-molecular-weight heparin (LMWH) subcutaneously. Immediately with heparin you need to give anticoagulation therapy with warfarin. The advantage of LMWH is that therapy is not necessary to be controled by laboratory testing and the therapy is for patients more suitable and finally the patient can be controled in out-patients ward. The treatment with warfarin is controled by determination of protrombin time, which is expressed in international units – INR ratio. The very important part of therapy is elastic compression of lower extremities by elastic bandage or elastic stocking II. compression class.
