Jak dávkovat perorální antidiabetika

Perorální antidiabetika (PAD) jsou indikována pro léčbu diabetes mellitus 2. typu (DM 2. typu). DM 2. typu je heterogenní onemocnění, na jehož vzniku se podílejí vlivy genetické i vlivy vnějšího prostředí. Manifestuje se u osob, které mají geneticky danou sníženou citlivost periferních tkání na inzulin – inzulinovou rezistenci (IR) a současně geneticky predeterminovanou určitou kapacitu B-buněk produkovat inzulin. Pokud produkce inzulinu nepokryje nároky IR, vznikne inzulinová deficience (ID). Důsledkem kombinace obou faktorů IR a ID je relativní nedostatek inzulinu a hyperglykemie. DM 2. typu se v čase vyvíjí, z počátečního stadia s převahou IR přechází do stadia s dominující ID. Stupeň zachované vlastní sekrece inzulinu zjistíme vyšetřením hladiny C-peptidu.
