European and american guidelines For the diagnosis and treatment of hypertension were published in the year 2003, followed by Czech and British guidelines in the year 2004. These guidelines are closly simmilar, but still small differences exist. The Czech Society for Hypertension has accepted the European classification with three pre hypertension grades (optimal, normal and high normal), three grades of hypertension and isolated systolic hypertension as a specific form. Combination of antihypertensive drugs and normotension are the basic recommedations for the treatment. A good control of the systolic blood pressure and other risk factors are requested....

Seasonal allergic rhinitis caused by pollen hypersensitivity belongs to the most frequent allergic diseases and its prevalence is increasing. In the Czech Republic it involves about 10% of population and it significantly influences the quality of life, school and work efficiency and also social involvement. The cost of its treatment is also not negligible. A complex treatment may significantly reduce the problems caused by this disease.

In surgery it is possible draw up fundamentals prophylaxis of venous thromboembolism this way: a) Mini heparin and low molecular heparin (LMWH) decline occurrence symptom – free and endeictic venous thrombosis and fatal pulmonary embolism when about 60% and decline all causes death. b) Mini heparin and LMWH have similar sensitiveness and safeness. c) In oncology has serve LMWH in higher dues. In longitude treatment venous thromboembolism with exert these fundamentals: a) near ill with heckled distal venous thrombosis is short – term secondary prophylaxis sufficient, b) secondary prevention longer than 6 month is fit near: active oncologic...

Natriuretic peptides, mainly type B (BNP) have become a useful tool for diagnosis and differential diagnosis of heart failure. Its plasmatic values have an independent predictive value for patient prognosis. Its physiologic effects, mainly vasodilatory, natriuretic, diuretic, and inhibition of pathologically activated vasoconstrictive neurohumoral systems theoretically determine them to the treatment of heart failure. Recombinant human BNP nesiritide (Natrecor, Scios, USA) applied in IV infusion was evaluated in several clinical studies in patients with acute or acutely decompensated chronic heart failure. It improved symptoms in patients, especially...

Wound healing is a complex process with participation of both systemic and local factors. A sufficient supply of substrates for regeneration processes is a necessary condition, in either form of sufficient stock or nutritional reserves. Special nutritional substrates are arginine, glutamine, antioxidant vitamins and zinc. Regeneration processes are suppressed during systemic inflammatory response, poor healing could be caused by mental stress. Local conditions favourable for healing require especially moisted environment secured by suitable macromolecular background. We reached surprisingly outstanding results in the field of wound healing with combination...

Since 1922, when insulin was first used to treat type 1 diabetes, we have witnessed an era of monocomponent (MC) insulins, later an era of human (HM) insulins, and currently we are witnessing an era of insulin analogues. Insulin analogues are derived from the HM insulin molecule in which a structural change in the sequence of some amino acids in chain B (or even in chain A) will be reflected in the pharmacokinetic properties. Short-acting insulin analogues, including lispro, aspart and glulisine, have a more rapid onset of effect and a shorter duration than standard soluble HM insulins and they also pose a significantly lower risk of both early and...

Oral antidiabetic agents are preparations used in type 2 diabetes mellitus treatment. There is important to manage both metabolic disorders in diabetic patients – impaired insulin sensitivity and dysfunction in insulin secretion as well. Basic tools in optimal metabolic control are preparations that influence mainly fasting glycemia (metformin, thiazolidindions) and mainly postprandial glycemia (acarbose, insulin secretagogues, incretin mimetics). Incretin mimetics work in more levels of glucose metabolism regulations and form a new quality in antidiabetic treatment.

Breastfeeding has a series of important advantages for both mother and child. It should never be interrupted prematurely without any important reason. The drug administration to breastfeeding mother brings often uncertainty whether and how the infant might be influenced. We can find the data on possible risk of a drug in the Summary of Product Characteristic. Other data come from published clinical cases of adverse reactions and studies of breastfed infants whose mothers took drugs. The decision about possible continuation of breastfeeding during drug administration depends on the ratio of evidence based or theoretically supposed risk and great advantage...
