Dyspepsia is a complex of various complains that a patient localizes in the region of gastrointestinal tract. Functional dyspepsia is a disorder not explained by structural and biochemical abnormalities. The etiology of lower dyspeptic syndrome has not been completely explained. The lower dyspepsia is usually divided to irritable bowel with subtypes (morning fractioned defecation, postprandial defecation and debacle), spastic constipation, functional diarrhoea, and incomplete and atypical forms. A thorough history targeted on characteristic signs and symptoms (a positive component) leads to the diagnosis and rules out an organic disorder (a negative...

Severe COPD consists of stage III. (severe) and stage IV. (very severe). Contemporary exact diagnosis of COPD severity is based on post-bronchodilator verified value of spirometric obstruction. Severe COPD stages are accompanied by the highest mortality, invalidity, hospitalization rate and decrease of health related quality of life. Severe stages of COPD are 10 percent of total COPD prevalence, but these 10 percent present the majority of direct COPD economic burden. The treatment of severe COPD is currently very expensive for the necessity of regular and long-term pharmacotherapy with inhaled long-acting bronchodilators, inhaled corticosteroids and...

Some infections caused by Chlamydiae have been known for a long time, e.g. trachoma, lymphogranuloma venereum and ornitosis, but they can be encountered neither in the Czech Republic nor in other developed countries or only rarely. On the other hand it has been shown over last ten years, that infection caused by Chlamydia trachomatis serovars D-K are the most frequent sexually transmitted urogenital infection. Chlamydophila pneumoniae has been known only for slightly more than 10 years, but it was found that with increasing age it asymptomatically colonizes the general population and cause community acquired pneumonia in probably more than 10% of cases....

The selection of an optimal treatment strategy for patients with diabetes type 2 is based on basic knowledge of fasting and postprandial blood glucose level and glycosylated hemoglobin. Postprandial blood glucose level correlates very tightly with risk of cardiovascular events. The insulin treatment in patients of older age with diabetes type 2 can be applied in several various regimes that are selected according all associated aspects. Current insulin strategies provide safe and very effective therapy.

A Raynaud´s phenomenon belongs to functional vessel disorders. It is a paroxysmal attack of vasoconstriction of peripheral parts of extremities caused by cold or emotion. Raynaud´s phenomenon can be classified as a Raynaud´s disease, if it presents without a known cause or a secondary Raynaud´s syndrome, if it presents as a part of some other underling disease. Secondary Raynaud´s syndrome may be caused by prolonged exposition of vibration while working with a manually operated pneumatic tools and vibration machines. The disease is considered to be an occupational disease. In years 1995–2006, 531 cases were acknowledged as occupational diseases...

Review of knowledge about ventricular premature beats contents definition, classification and incidence of ventricular premature beats. It is necessary to postulate the seriousness of ventricular premature beats for its treatment. The seriousness is higher when structural heart disease is present, with lowering of left ventricular function and with higher number or complexity of premature ventricular beats. It is necessary to perform echocardiography and holter ambulatory ecg monitoring to postulate the seriousness of ventricular premature beats and according to the results the correct treatment method to postulate. Proarrhythmogenic effect of antiarrhythmic...

The author presents some basic facts about stress incontinence, its aetiology, prevalence, and risk factors. He mentions basic diagnosis, which every physician is capable of, as well as special diagnosis which requires the knowledge and skills of a specialist. An overview is given of the methods of conservative treatment the most effective of which are, according to the author‘s experience, electrostimulation and pelvic floor exercises. Operation outcomes for Burch colosuspension and TVT procedure (Ulmsten) in the long term are reported. At present, operational treatment most typically involves miniinvasive urethropexy performed mostly by using...

Phytoestrogens are natural active substances found in numerous plants. They structurally and functionally exhibit estrogen-like biological activity and thus can influence the condition of human organisms. Phytoestrogens are mainly recommended for treatment of climacteric and menopausal symptoms where offer a convenient alternative to HRT. It is not generally known that phytoestrogens can influence pharmacokinetics and pharmacodynamics of some remedies through induction or inhibition of liver and intestine enzymes. Risk of possible interaction can be decreased by the detailed medical history. Phytoestrogens also exhibit indispensable antioxidant qualities....
