Portální hypertenze je stále závažnou komplikací zejména pokročilých jaterních onemocnění. Komplikace portální hypertenze a to zejména krvácení z jícnových varixů je bezprostředně život ohrožující akutní stav. Transjugulární intrahepatální portosystémový zkrat (TIPS) je novou metodou, která vytváří portosystémový zkrat katetrizační technikou. Ve svých důsledcích vede ke snížení portálního tlaku a tedy k zástavě krvácení do zažívacího traktu, resp. snižuje riziko recidivy krvácení zejména u nemocných s portální hypertenzí na podkladě závažného jaterního postižení. TIPS je tedy indikován v léčbě akutního krvácení a v sekundární prevenci krvácení z jícnových varixů, je však využíván i v léčbě dalších komplikací portální hypertenze.

TIPS IN THERAPY OF PORTAL HYPERTENSION

Portal hypertension is a major sequelae to severe liver disease. The complications of portal hypertension, which are mainly haemorrhage from esophageal varices, can be of an acute life-threatening form. Transjugular intrahepatic portosystemic shunt (TIPS) represents a new technique which can create a portosystemic shunt via a catheter. The outcome of this procedure is a decreased portal pressure, which leads to the cessation of gastrointestinal bleeding. In patients with portal hypertension in the advanced stages of liver disease, the risk of recurrent bleeding is thus diminished. TIPS is indicated in the treatment of acute bleeding and in the secondary prevention of recurrent variceal haemorrhage and could also be used in the management of other complications of portal hypertension.
