The goal of treatment of coronary heart disease is relief of symptoms and cardioprotection., i.e. measures leading to protection of myocardium and decrease or prevention of cardiac damage and improvement of prognosis. Cardioprotection is a general pathophysiologic term including protection of myocardial cells from the damage by ischemia and consequent impairment by reperfusion, inflammation, hypertension and toxic and degenerative changes including several types of apoptosis. Cardioprotection has many specific forms in clinical practice, different in acute situation, different in chronic patients or in patients with subclinical atherosclerosis (tab 1).

Allergic rhinitis is the most common of atopic diseases and has been estimated to affect up 10–30% of the population. Although rhinitis was once considered to be a medical condition of trivial importace, now is recognized to affect quality of life and impair physical a cognitive functioning. Allergic rhinitis should be considered as a possible concomitant diagnosis in all patiens with bronchial astma, recurrent otitis, chronic sinusitid and nasal polyposis. The same inflammtory mechanisms and inflammatory cells are prezent in nasal and bronchial mucosa. Appropriate antiinflammatory therapy may reduce morbidity associated with these diseases.

Biventricular stimulation is a new non-invasive method of treatment of severe congestive heart failure. Left bundle branch block or non-specific intraventricular conduction delay is present in up to 30% of patients with severe heart failure and by dyscoordinating left ventricular contraction lead to further deterioration of its systolic function. The authors summarize pathophysiological mechanisms of unsynchronized left ventricular contraction and present survey and major conclusions of the most important clinical trials published to date on the field of cardiac resynchronization.

Growth hormone has important metabolic effects and its secretion is lower but continues in adulthood. The development of recombinant growth hormone in the mid-1980s, has led to the description of adult growth hormone deficiency syndrome. It consists of mental impairment (decreased well-being and energy, increased anxiety and social isolation), abnormal body composition (increase of visceral fat and decrease of lean body mass), decreased bone mineral density with increased fracture risk, impairment of heart functions and a number of abnormalities (lipid profile, insulin resistance, endothelial dysfunction) predisposing to premature atherosclerosis....

The article introduces overview of the most frequent skin manifestations of internal diseases, in which skin manifestations may appear during the course of the disease, so called dermadromes. The complicated relationship among disorders of internal systems and their skin symptoms is emphasized. The skin manifestations in serious systemic disorders and paraneoplastic syndromes in neoplastic diseases are described in detail.

Colorectal cancer represents serious complication of inflammatory bowel disorders (IBD), Crohn´s disease and ulcerative colitis. Colorectal cancer is associated with each sixth deaths in IBD patients. The authors in this review summarise the current opinion on the pathogenesis and early diagnosis dysplastic lesions and colorectal cancer. There are discussed risk factors which induce to the next development of dysplasia. The most important are: long term course of disease and extensive involvement of the large bowel. The extraordinary risk of colorectal cancer is asscociated with primary sclerosing cholangitis

Malaria represents one of the leading causes of mortality in tropics, especially in sub-Saharan Africa with more than 1.5 million deaths per a year, mostly in young children. The aim of this review was to provide comprehensive information on the infection, including biology of malaria parasites, clinical presentations of the disease and diagnostic tools. The imported malaria cases to the Czech Republic are summarized. Current therapeutic and profylactic approaches and antimalarial drugs available in the Czech Republic are also reviewed.
