In the treatment of pancreatic diabetes is important to assess the need of insulin treatment, risk of hypoglycemia and microangiopatic complications. Insulin treatment of pancreatic diabetes is not necessary in case of sufficient residual insulin secretion and good control of diabetes by oral hypoglycemic agents. It is necessary to stop alcohol intake in patients with chronic alcoholic pancreatitis. Early endoscopic or surgical treatment of stenosis or oclusions of pancreatic ductus is important. Further destruction of pancreatic tissue might be prevented by this way and its complication, for example a need of insulin treatment, higher risk of hypoglycemia...

Diabetic foot syndrome is one of the most serious long-term complication of diabetes mellitus. The healing of foot ulcers is limited by multiple factors and therefore requires a multifactorial approach. The reduction of pressure to the neuropatic foot ulcer is essential for treatment and total contact cast is the standard method. In the management of ischaemic ulcers the revascularisation should be performed at first. Effective debridement including biological methods (larval therapy) in the treatment of local chronic wounds has been long established. Hydrosurgery using the „Versajet“ system is a latest method indicated for debridement....

The definition of metabolic syndrome is clinical and the metabolic syndrome is not identical with the insulin resistance syndrome (Reaven’s syndrome). It’s importance lies in recognizing all pathophysically jointed metabolic disturbances, their full assessment and management. The concept of metabolic syndrome shed light the relevance of body fat distribution and catalyzed the transition from the quantitative to “qualitative” conception of obesity.

The frequency of invasive fungal infections has significantly increased over the last two decades with the rise in at-risk populations of patients. The morbidity and mortality of deep fungal infections are high and management of these infections is difficult. The epidemiology of invasive fungal pathogens has also changed. Non-albicans Candida, Aspergillus spp. and moulds other than Aspergillus have become increasingly recognised causes of invasive diseases. Recently developed antifungal agents, such as second-generation triazoles and echinocandins, provide the potential to improve management options and therapeutic outcomes of these infections. This...

Colorectal cancer is a serious health problem in our republic. Compared with the worldwide situation, we belong to countries with the highest incidence of these tumours and mortality from them. Moreover, no efficient screening takes place in our country, therefore despite certain improvements the treatment outcomes have not been still good. The article reviews diagnostic and therapeutic options in tumours of the colon and rectum. The importance of prevention and population awareness of risk factors and first symptoms of the disease and necessity of timely medical examination are pointed out. Multidisciplinary team collaboration of specialists –...

Herpesviruses which affect man occur ubiquitously in all populations throughout the world. It is a characteristic feature of herpesvirus that they generally remain in a latent form after clearance of the primary infection. The overall majority of clinical problems are related to activated latent infections, eg., recurrent lesions of herpes simplex labialis or herpes genitalis and in particular herpesvirus infections and reactivation in immunodeficient individuals. The review concerns the more common recurrent clinical manifestations of herpesvirus simplex infection (HSV 1, 2) in man and a new approaches to the prevention and therapy.

Immune system of the lung protects an area of about 100 m2 and comes into contact with 10 000 L of inhaled air containing numerous particles and infection agens daily. In this respect, the co-ordination between the mechanisms of innate immunity and adaptive immune responses is particularly important. Epithelial cells represent the first defense barrier by their mucocilliary transport but produce also multiple chemokines regulating the influx of other immune cells and release cytokines inducing their morphological a functional properties. Alveolar macrophages originating from blood monocytes are probably the most potent weapons of innate immunity in...

In this article, the basic concepts used in conceptualizing stress and classifying its related phenomena are defined and a general conceptual framework for understanding the interrelationships among these concepts is proposed. The mechanisms underlying Cannon and Selye stress are discussed between stress and physical health. The notion of a stress system as the effector of the stress syndrome is suggested, and its physiologic and pathophysiologic manifestations are described. The diffrence between the eustress and the disstress are elucidated. The coping mechanisms and the personality factors especially D-personality are mentioned. The conceptual...
