Oral anticoagulant therapy has become a cornestone in many internal cases. As the population as a whole is aging, the number of patients who need warfarin is likewise increasing. The main problem of this therapy is due to the difficulty in determining the correct dosage of the drug to obtain effective anticoagulation. Altough there are miscellaneous reasons for this variability a great emphasis is often incorrectly placed on food interaction. The article provides an overview of several possible causes of interindividual variability in warfarin dose and their clinical importance.

The aim of the article is to provide a basic information about immunosuppressive drugs and their use in cardiology. The most frequent indication for immunosuppressants is the prevention and treatment of rejection after cardiac transplantation. Drugs with antiproliferative effect (m-TOR inhibitors) are also the most frequent ones used in drug-eluting coronary stents, decreasing significanty the rate of restenosis. Another form of coronary proliferative syndrome, chronic rejection after cardiac transplantation, can be effectively influenced with systemic aplication of these drugs. Another indication for immunosuppressives present some types of...

With the advent of novel direct thrombin inhibitors (dabigatran) or factor Xa inhibitors (rivaroxaban, apixaban, etc.), a need has emerged to critically evaluate the role of established indirect factor Xa inhibitors and thrombin inhibitors, low molecular weight heparins (LMWHs), and to redefine their position among the other anticoagulants. Given the rapid onset of action, a reliable effect, the possibility to be administered once or twice daily and lower treatment costs, LMWHs remain to be effective drugs in the prophylaxis of thrombotic complications during procedures involving weight-bearing joints. For this indication, rivaroxaban and apixaban...

We have been drinking wine at least for 10 000 years or longer, in our region already from Celtic and Germanic times. Favorable effects on prevention of myocardial infarction, stroke, diabetes and hypertension were demonstrated recently, following attempts to use alcohol for treatment in ancient times. Favorable effects of wine drinking could be seen also in aging population. The best results for men are reached by daily drinking of 20–40 g alcohol at dinner. A moderate dose for women represents half of the dose for men.
