in management of serious hypertension Sympathetic nervous system plays significant role in pathophysiology of hypertension. New method in its management is cathether- based radiofrequency ablation of the sympathetic nerves located around renal arteries. Comparing to dilatation of renal artery stenosis renal, renal denervation (RDN) shows promising long lasting decrease of blood pressure. RDN is indicated in resistent hypertension (three different antihypertensive drugs icl. diuretic are not effective in correction of serious hypertension) with a risk of organ damage following heart and kidney failure. It is supposed, RDN could be effectively...

Clostridium difficile is the main cause of nosocomial diarrhea in industrialized countries. Clostridium difficile infection (CDI) is a potentially fatal illness with an increasing incidence worldwide. Despite intensive ongoing research into CDI treatment, management of CDI still poses serious problems, such as high probability of recurrence and refractoriness to treatment.

Self-monitoring of blood glucose is an essential component of a safe diabetes treatment and helps patients with diabetes to manage their diabetes efficiently. It also provides valuable data about the diabetes control for both patients and healthcare professionals. Analysis of glucose profiles can support decision making and choosing appropriate treatment strategies. Personal blood glucose meters opened a way for reaching and keeping good diabetes control effectively and for cutting down the risk of late diabetes complications. In patients with type 1 diabetes, the precision of blood glucose meters is critical. These patients are usually very...

The growth hormone deficiency in adulthood became a separate clinical entity in 1997. Then the growth hormone therapy gradually penetrated into the clinical practice. Many questions was raised as for its effects despite increasing experience and length of time, of course. As it turns out, not all of them have been fully answered, up till now. Anyway, appropriate selection of patients suitable for the growth hormone therapy in adulthood should be strictly demanded. An original scheme under which it is possible to set up the appropriate replacement dose to the patient exists. Recent years gradually have dissipated doubts stemming from the possibility...

The treatment of stable angina pectoris (AP) based on affecting cardiomyocyte metabolism has been under investigation for a number of years. New approaches in improving and optimizing cardiac energy metabolism by pharmacologically intervening in a complex cascade of events during ischemia at various levels are promising methods to protect cardiomyocytes. The basic principle involves inhibition of fatty acid oxidation and stimulation of glucose metabolism. Also available, however, are other strategies, such as affecting ATP storage by directly interfering with nucleotide metabolism. In the Czech Republic, trimetazidine is the most widespread...

Diabetes mellitus and heart failure are nowadays both diseases having an epidemic characteristics. Diabetes seems to be an engine of heart failure development. More severe heart failure forms show more frequently the presence of diabetes. This is true for both systolic and diastolic heart failure types. In the pathophysiology of heart failure in diabetics there is an influence of myocardial ischaemia, of meta bolic (lipidic and diabetic) serum changes and of diabetic cardiomyopathy. Also activation of the renin- angiotensin-aldosterone system is important: mainly the alternative pathway where enhanced activity of ACE-2 enzyme seems to be present....

Case report describes a patient with accidentally discovered hypoechogenic liver mass diagnosed by abdominal ultrasonography and confirmed by CT scan and also the relationship to the wall of the duodenum was founded. The patient was examined and gradually the malignancy, abscess or an inflammatory disease of the duodenum with the liver fistula were excluded. Esophagogastroduodenoscopy was done and the deep duodenal bulb ulcer with penetration into the liver was detected. It was decided about conservative therapeutical approach with the deployment of the maximum oral dose of proton pump inhibitors with full condition restitution. The patient...

Aim: The aim of the thesis is to lay out a detailed analysis of the probable association of postural stability with a child obesity. Methods: The used methods were compilations and comparisons of the available expert literature. Furthermore, the authors‘ experience in current therapeutical field were also used. These were thoroughly examined within a tight group discussion. Results: The thesis proved the negative impact of obesity on the postural stability of children suffering from obesity. The authors rightly recommend the examination of the postural stability as part of the overall medical evaluation of an obese child. This result...

Perioperative management of patients on anticoagulant therapy involves assessing and balanci ng individual risks for thromboembolism and bleeding. Discontinuing peroral anticoagulant therapy is usually necessary for most surgical procedures but increases the risk of thrombotic events. Temporary periprocedural substitution of low-molecular-weight heparin or unfractionated heparin in place of warfarin, is an effective means of reducing the risk of thromboembolism but may increase the risk of bleeding. The timing of warfarin withdrawal and timing of the preoperative and postoperative apllication of parenenteral anticoagulant are critical to balancing...
